The NHS does not pay for some of the services provided by your Doctor. Fees are charged for this work as recommended by the British Medical Association. These include private sick notes, accident or sickness insurance forms, holiday cancellation forms, private healthcare claim forms, medicals for HGV, sports, driving etc and short reports/letters. A list of our charges can be found in our waiting room. The fees have been reviewed based on the time it takes it completes the request including both admin and clinical time.
Please note, when a Doctor signs a certificate, completes a report or writes a letter, they will need to check the patient’s entire medical record to ensure that they are providing accurate information. Doctors will not be able to complete forms same day, so please ensure that you give them reasonable notice. With this in mind please allow a minimum of 7 working days for your form/letter to be completed. We will contact you when your from is ready to be collected.
Please note that payment must also be made in advance. You can pay over the phone, online bank payment (if you use this option you will need to provide proof of payment), or cheque (made payable to Drs Adey & Dancy). Payments for medical examinations will need to be made before the appointment can be booked.
From 1st February 2025 we will become a cashless practice so we will be unable to take this method of payment.
